Wedding Facility Use Request Form (Non Member)

Break Down of Cost

Payment for all costs, will be due no later than 30 days before the wedding

 

Seating and Tables Available for use:

153 Chairs 

15 Round tables

17 Long tables 

 

Any and all stage equipment will stay in place or be moved by staff

Ceremony (includes rehearsal):

  • $500 Room rental

  • $100 cleaning service 

  • $25/hr for sound engineer (needs to be pre-approved by church staff) 

Reception:

  • $500 for room rental (recommended is the community room in the basement)

  • Responsible party will need to arrange for catered food.

  • No alcohol use permitted on church property

By typing my full name below, I agree:

1 - To only use the designated areas specified in this agreement.

2 - To absolve The Well, its leadership and members of any liability for personal injury to any individual resulting from the use of the church facility.

3 - To take full responsibility for any property damage that results during the use of the facilities. Please report any damage to the church staff promptly.

4 - I also understand that I am fully responsible for set up, clean up and return to normal set up of the facility. 

5 - I will not provide or partake in use of alcoholic beverages on church property

Responsibilities for and after Building Use:

1. If food or drink is involved, wipe all tables clean. If stains occur please notify staff.

2. Set up and return all tables, chairs, and other equipment to their proper places after your event. Return all rooms used to their normal set up.

3. If using the kitchen, please wash and dry all dishes and return them to the correct cabinets. Take all extra food and beverages with you, unless specific plans for usage have been made with The Well staff. Wipe all counters and leave the kitchen clean and ready for the next use.

5 - No use of real candles or other flammable items
