Code of Conduct Agreement

I will uphold and abide by the beliefs and core values of The Well.
I will take ownership of my role and position at The Well Church and will practice good stewardship of The Well's resources.
I will handle conflict in a civil and Christ-like manner.

I have read the above and agree to follow The Well's code of conduct.

Background Check Consent and Disclosure

The Well Church will not employ, independently contract with, or grant direct access privileges to an individual until a criminal history check is conducted.

By completing this form, I hereby consent to and authorize The Well Church to conduct a background check that includes a search of state and federal databases. I understand that this consent extends to the release and sharing of such information with the Michigan Department of Human Services and State Police.
I understand, except for a knowing or intentional release of false information, The Well Church has no liability in connection with a background check conducted or the release of criminal history record information for the purposes of making a decision.
I understand that The Well Church, in denying a position to serve, and reasonably relying on information obtained through a background check, is provided immunity from any action brought by an applicant due to the decision.
I understand that I may request a copy of the results whether or not The Well Church determines it necessary to utilize me or my services.
